Contacting us does not establish care
Submitting a contact form, requesting an appointment, or communicating with us through the website does not establish a patient–provider relationship or guarantee an appointment. A patient–provider relationship begins only after Encompass accepts you for care and you complete the applicable intake process.
Please do not use the website’s contact form to send detailed medical history or other sensitive health information. If you are an established patient, use the communication method our office has provided for your care.
Emergencies and urgent concerns
This website and its contact form are not monitored for emergencies. If you are in immediate danger or need emergency medical or psychiatric care, call 911 or go to the nearest emergency department. In the United States, you can also call or text 988 for the Suicide & Crisis Lifeline.
Do not use the website to request an urgent medication refill or report symptoms that need an immediate response.
Patient services and outside platforms
Encompass may link to a patient portal, forms, telehealth services, or other platforms. Those services may have their own terms and privacy notices. Your care is also governed by the policies and consent forms provided during intake. These website terms do not replace those documents.
