United, Select Health, CIGNA, Blue Cross Blue Shield (Regence), EMI, PEHP, Motiv, Aetna. OON for DMBA. Check with your specific plan before your appointment to make sure. We do not accept Medicaid at this time.

We can do Superbills for Out-of-Network, we also offer self pay.

Intake: $250, Follow up: $150

What to Bring

• Your insurance card (if applicable)

• A valid photo ID

• Payment method for any co-pay, deductible, or self-pay balance
